How Patients Enroll

Enrollment happens during your healthcare visit. Here's the process:

  1. Provider Explains Service: Your healthcare provider will tell you about appointment reminder text messages and ask if you'd like to enroll.
  2. Provider Reads Consent Script: If you're interested, your provider will read you the consent statement below.
  3. You Respond Verbally: You simply say "Yes" or "No" to indicate your choice.
  4. Confirmation: If you say yes, you'll receive a confirmation text within 24 hours.

Your healthcare provider will read this to you:

"Would you like to receive text message reminders for your appointments and health information? If yes, please provide your mobile phone number. You can opt out at any time by replying STOP to any message. Standard message and data rates may apply."

Your verbal response: "Yes" or "No"

If you say yes, your provider will enter your phone number into the system.
